L-Theanine
Nine small trials, none in a diagnosed anxiety disorder, and one null pooled estimate.
What Its Legal Status Actually Means
Read this before anything below it. Legal to sell is not the same fact as reviewed, verified, or approved.
L-theanine is sold in the US as a dietary supplement under the Dietary Supplement Health and Education Act of 1994 (DSHEA), and as a food additive it holds Generally Recognized as Safe (GRAS) status for use in beverages. Neither of those is an efficacy approval: the FDA does not review L-theanine supplements for effectiveness before sale, does not verify that a capsule contains the dose on the label, and has never approved L-theanine to prevent or treat anxiety, stress, insomnia, or any other condition. Branded forms used in research (Suntheanine from Taiyo Kagaku, AlphaWave from Ethical Naturals) are proprietary preparations sold by the same companies that funded the trials testing them.

Read this first: A null result across a wide credible interval means "not proven to work", not "proven not to work" — the L-theanine arm of this network rests on few small trials, so a modest real effect cannot be ruled out. The analysis came from a traditional Chinese medicine research institute, which if anything would bias it toward finding herbal benefits, making the null finding for L-theanine harder to dismiss.
In a Bayesian network meta-analysis of 29 randomized trials of 12 medicinal herbs in adults with diagnosed or subthreshold anxiety, L-theanine did not beat placebo: the effect on the Hamilton Anxiety Scale was -0.49 points with a 95% credible interval of -6.54 to 5.57, which straddles zero in both directions. This is the only pooled quantitative estimate of L-theanine for anxiety found in the literature, and it is null.

Read this first: The review did not pool the trials or grade certainty of evidence, so "may assist" is a narrative judgement rather than a measured effect; none of the nine trials studied people with a diagnosed anxiety disorder, and several were manufacturer-funded.
A PRISMA-guided systematic review found only nine peer-reviewed randomized controlled trials of pure supplemental L-theanine against a control for stress or anxiety in humans. The authors concluded 200-400 mg per day "may assist" in reducing stress and anxiety in people exposed to stressful conditions, while stating plainly that longer-term and larger-cohort studies are needed before L-theanine can be clinically justified as a therapeutic agent. Read this first: Funded by Taiyo Kagaku Co., Ltd., which manufactures the Suntheanine-brand L-theanine; two of the seven authors are employees of that company and it supplied both the active and placebo tablets. Several outcomes were measured within-person before-and-after rather than against placebo, which inflates apparent effects.
Thirty healthy adults with no major psychiatric illness took 200 mg/day of L-theanine or placebo for four weeks each in a crossover design. Depression, trait anxiety, and sleep quality scores all fell after the L-theanine period (p=0.019, 0.006, and 0.013), and verbal fluency improved. With 30 participants, a four-week exposure, and no diagnosed condition among them, this trial can suggest a direction but cannot establish that L-theanine treats anything.

The tested product was a multi-ingredient drink, not isolated L-theanine, so the effects cannot be attributed to L-theanine alone.
Thirty-four healthy adults aged 18-40 drank an L-theanine-based nutrient drink or placebo before a multitasking cognitive stressor. Self-reported stress one hour after dosing was lower on the active drink, and the salivary cortisol response was lower three hours after dosing; no cognitive performance changes were seen at all, and the brain-wave (alpha) changes appeared only in participants higher in trait anxiety and did not correlate with either the stress or cortisol results. This is a single-dose laboratory stress test in healthy volunteers, not a treatment study.

With 16 participants and one participant lost after period 1, the trial is far too small to support the conclusion in its own discussion that the product "should be considered as a nutraceutical supplement to manage acute stress."
Sixteen moderately stressed healthy adults received a single 200 mg dose of branded L-theanine or placebo before a mental-arithmetic stress task. Frontal and whole-scalp alpha brain-wave power was higher three hours after the active dose and salivary cortisol fell more one hour after it. Sixteen people, one dose, and brain-wave and hormone readouts rather than any clinical outcome — this is a mechanism demonstration, not evidence that L-theanine reduces anxiety in daily life.

Nobody checks this purchase the way a prescription is checked. No prescriber reviews it against the rest of what you take, no pharmacist screens the interaction, and no regulator confirms the dose in the capsule before it is sold. That is why this section sits here at full length rather than as a footnote.

L-theanine looks benign. Trials at 200-400 mg per day for up to four weeks report no adverse events beyond placebo, and the amino acid occurs naturally in tea, so most people have been consuming small amounts for years. The honest problem is not safety, it is that the evidence is thin to the point of being uninformative: the trials are tiny (16 to 34 people), short (single dose to four weeks), almost entirely in healthy or mildly stressed volunteers rather than anyone with a diagnosis, and predominantly paid for by the companies selling the ingredient. The one pooled quantitative estimate available found L-theanine no better than placebo for anxiety. Because it is mildly sedating in some people, combining it with alcohol, sedatives, or blood-pressure medication has not been studied and is a reasonable thing to ask a pharmacist about. Nothing here establishes that it treats an anxiety disorder, and nobody should substitute it for treatment that does.

- No trial of L-theanine in people with a diagnosed anxiety disorder was found in PubMed. That absence is the story: every entry below studies healthy or self-reported-stressed volunteers.
- No meta-analysis of L-theanine alone exists; the network meta-analysis below is the only pooled quantitative estimate found, and it is null.
