What the best-paid psychiatrists publish about the drugs that pay them

this page does not question a single scientific finding. it asks a simpler question that the reader of a journal is almost never helped to ask: when a leading psychiatrist writes about a drug, is the drug’s maker paying them, and which way does the paper come out? we took the federal payment records and the published papers, put them side by side, and read every conclusion.

what this page is not. it is not a claim that any paper is wrong, that any author was told what to write, or that anyone broke a rule. every payment was legal and was reported to the government by the company. several of these authors are the investigators who ran the drugs’ trials, and explaining those results is what one would expect them to do. the point is narrower: the reader is rarely shown the relationship, and the pattern is too consistent to call random.

1. which way the papers come out

ten of the twenty best-paid psychiatrists publish. together they wrote 451 papers from 2019 to 2026. 129 of those are about a branded drug whose maker paid that same author at least $10,000 between 2021 and 2024. we read the concluding sentences of every one.

set aside the 29 neutral papers (mechanism studies, pharmacokinetics, trial designs, corrections) and 100 remain that reach a verdict on the drug. 93 of them, 93%, are favorable. 6 are mixed or cautious. 1 reports a trial that did not meet its goal. not one concludes that the drug does not work or questions whether it should be used. 19 of the favorable papers go further and argue for a new use, a new group of patients, or using the drug earlier. 82 of the 129 papers, 64%, list a co-author employed by a drug company.

2. is the overlap random?

ten authors and 23 branded drugs make 230 possible pairings. where the maker had paid the author $10,000 or more for the drug, the author had published on it 42% of the time. where it had not, 15%. of all their papers, 163 name a brand still under patent and 48 name only generic drugs, in a field where the twenty promoted brands are 2.3 percent of prescriptions.

3. three jobs, one small group

the twenty best-paid psychiatrists, by what they do besides speak. 9 publish on drugs whose makers pay them, 8 hold a position that shapes what other psychiatrists learn (a conference committee, an education company, a professional society or a trade journal), 5 ran industry-paid trials in 2024, and 8 do none of these.

every one of the twenty is a paid speaker: together, 2,622 separate speaker fees in 2024 and $43.1M over four years. the circles show what else they do. the ones who write, lead and run trials are a minority, and the same names fall in more than one circle.

that is the structure worth seeing. a company pays an investigator to run a trial, the investigator publishes the result and its follow-up analyses, speaks about it a hundred times a year, and helps set the programme of the conference where colleagues learn about it. each step is ordinary. together they mean a small group of paid people produces much of what the rest of the field reads and hears. the marketing page has the prescribing side, and the payment list has every name and amount from the federal file.

4. the other explanations, which may be the right ones

none of that makes the relationship unimportant. it is why it matters that the reader can see it. a consensus statement recommending a drug as first-line treatment reads differently when you know its authors are paid by the maker. the information is public. almost nobody is shown it.

5. every paper, so you can check our reading

each row links to PubMed. “paid” is the amount Open Payments shows that author received in connection with that drug from 2021 to 2024. the verdict is ours, from the abstract’s conclusion.

Questions people ask

Does this prove the research is biased?

No. It shows that 93 of 129 papers by these authors about drugs whose makers pay them reach a favorable conclusion and none concludes the drug does not work. That pattern has at least two explanations that do not involve anyone shading a result: companies recruit the investigators who ran a drug's successful trials, and journals rarely publish a paper whose finding is "nothing happened". Cochrane's review of industry sponsorship found sponsored studies reach favorable conclusions more often even when their methods are sound. The relationship is what readers are not shown. The findings may all be correct.

How were the papers judged favorable or not?

One reader read the concluding sentences of each abstract and sorted it into four groups: favorable to the drug (it works, is well tolerated, is cost-effective, or should be used), neutral or descriptive (mechanism, pharmacokinetics, a trial design, a correction, no abstract), mixed or cautious, and reporting a negative result. It is a judgment, and a different reader would move a few papers between groups. Every paper is listed below with its PubMed link so anyone can check.

Why only ten psychiatrists?

Of the twenty paid most by industry in 2024, eight have little or no research record in PubMed and two have names too common to search reliably. The ten here are the ones whose publications can be identified with confidence. The eight who do not publish are paid almost entirely to give talks.

Is it wrong for a researcher to be paid by a drug company?

It is legal, common and disclosed. Most new drugs are tested in trials the maker pays for, and the clinicians who run those trials are the natural people to explain the results. The question this page raises is about the reader, not the writer: a psychiatrist reading a consensus statement or sitting in a lecture is rarely told, in plain terms, that the author received several hundred thousand dollars a year from the maker of the drug being recommended.
